Found a 3-inch gap in the root pass on a 200-gallon tank at a chemical plant near Baton Rouge, and the owner said it passed inspection 3 years ago - how the hell does that get missed?

Did they even look at the weld? Sounds like the inspector just signed off without cracking a book. Some of these guys are literally stamping anything. That's how tanks blow up.
