The 2AM warehouse shift that made me stop trusting my gut

I used to work the overnight shift at a distribution hub off I-95 in Richmond, and we had this rule about double checking pallet weights before they hit the truck. One night I skipped it on a load going to Norfolk because I was tired and the label said 1,200 pounds, so I waved it through. Turned out the forklift operator had swapped in a pallet that was actually 1,800 pounds, and we blew out a rear tire on the interstate about 40 minutes later. Has anyone else had a similar close call where that extra 30 seconds would've saved a whole mess?
