Three weeks to fix a payroll typo that took 10 minutes to cause

I work in a small office and messed up a decimal point on someone's hours back in March. The error went through payroll and the guy got paid about $400 short. It took me three weeks to get it sorted because the payroll company kept saying it was my fault and my boss kept putting off the call. In the end I had to write a formal email to the state labor board just to get them to take it serious. The actual fix once someone finally listened was maybe 10 minutes on the phone. Has anyone else had a tiny mistake turn into a full month of headaches?.
