Base plate anchor layout, my 3AM math was off by 2 inches

Last Wednesday I was setting anchor bolts for a steel column on a job in Austin and got in a hurry with the tape. I marked my grid at 2 AM because the concrete crew showed up early, and I eyeballed the diagonal instead of actually squaring it. The bolts came out 2 inches off center and we had to redrill and epoxy them, which added a whole day to the pour schedule. Lesson learned, I now use a string line and a framing square before I even touch the chalk. Has anyone else had a layout screwup that cost you a full day?
