But how often does a rushed dig actually cause a real problem? Sometimes speed is necessary to meet a deadline without issues.

Actually, I see it the other way. Rushing digs is how you get those big, expensive problems (like hitting a utility line). The short-term speed just isn't worth the long-term risk and cost.
