Heard a PM admit they fudged soil test results on a commercial job near Austin

I was grabbing coffee last Tuesday and overheard a project manager from a mid-size firm bragging about altering compaction reports to avoid delays on a 12-story foundation. He said they saved 3 weeks but didn't mention the risk of settlement cracking later. Has anyone else dealt with falsified paperwork on your sites and how do you catch it before it's too late?
