Crisis tabletop drill went sideways, now CEO wants monthly ones

Ran a fake data breach drill with 12 senior folks in Chicago last month and the comms plan fell apart in 20 minutes because no one knew who approved external statements. The CEO about had a fit and is pushing for monthly practice runs now. Anyone else's leadership realize their gaps only after a mock scenario flopped?
