My friend's old warehouse job had a similar thing happen. His supervisor made him bypass a sensor on a conveyor belt to keep things moving during a holiday rush. The sensor was jammed but instead of fixing it, they just taped it down. A pallet got misaligned later and nearly crushed someone's foot.

At the plant, we always fixed sensors immediately.

Oh man that's terrifying. I read a news story last year about a factory where they disabled a safety gate on a machine for the same reason, to not slow down production. A worker got his arm pulled in and it was really bad. It's crazy how often that happens just to save a few minutes. Management never seems to learn until someone gets seriously hurt.
